We are seeking a Intake Coordinator to manage the referral and admission process for home health, hospice, and private duty nursing services. This role serves as the gateway between referral sources and clinical teams, ensuring patients are assessed, authorized, and scheduled for services efficiently and compliantly.
Home health intake is where the patient relationship begins. A hospital discharges a patient who needs home nursing. A physician orders skilled therapy. A family calls looking for help with a loved one. You are the first person they reach, and your ability to collect the right information, verify the coverage, and coordinate the first visit determines whether that patient gets the care they need without a gap. Speed, accuracy, and compassion define this role.
Responsibilities:
- Receive and process patient referrals from hospitals, physician offices, skilled nursing facilities, and case managers
- Verify insurance eligibility, obtain authorization for home health services, and confirm coverage parameters
- Collect clinical information from referral sources to determine patient acuity and appropriate service levels
- Coordinate with scheduling and clinical staff to assign cases and arrange initial home visits within required timeframes
- Ensure all intake documentation meets Medicare, Medicaid, and commercial payer requirements for home health admission
- Maintain referral tracking logs and report on conversion rates, referral sources, and turnaround metrics
- Communicate with patients and families about services, scheduling, and what to expect during the first visit
- Support compliance with CMS Conditions of Participation and state home health licensure requirements
- Verify face-to-face encounter documentation and physician certification requirements for Medicare home health admissions
- Coordinate with hospital discharge planners and case managers to ensure seamless transitions of care
- Process urgent and same-day referrals with expedited intake workflows
- Track and resolve authorization denials and payer coverage issues that delay admission
- Maintain knowledge of Medicare home health eligibility criteria, OASIS requirements, and PDGM payment model
- Support census management by monitoring referral volume, admission rates, and capacity across service areas
Qualifications:
- High school diploma required; associate degree in healthcare or related field preferred
- Minimum 2 years of experience in home health intake, medical office referral coordination, or healthcare customer service
- Knowledge of Medicare home health eligibility criteria, OASIS assessment requirements, and face-to-face encounter rules
- Experience with home health EMR systems (Homecare Homebase, Axxess, MatrixCare, or similar)
- Strong phone presence with ability to build rapport with referral sources quickly
- Excellent multitasking skills in a fast-paced, high-volume referral environment
- Understanding of HIPAA privacy requirements in home health documentation
- Organized and detail-oriented with ability to manage multiple concurrent referrals
Schedule:
- Full-time position
- Standard business hours
- Standard full-time hours per pay period
Salary Range: The salary range for this position is approximately $40,000 – $50,000 annually ($19.23 – $24.04 per hour), based on experience and qualifications. Relocation assistance may be available for qualified candidates.
