Job Description

Location: Chicago, IL (Onsite)

We are seeking a Complex Case Manager, Licensed Clinical Social Worker (LCSW) to support high-risk members within a multidisciplinary care management environment. This role is responsible for conducting psychosocial assessments, coordinating behavioral health and community-based services, and developing individualized care plans that address both clinical and social needs.

This position is ideal for a licensed social worker with managed care, behavioral health, or population health experience who is passionate about helping individuals navigate complex healthcare and social systems while improving long-term outcomes.

Responsibilities:

  • Conduct comprehensive psychosocial assessments for members with complex medical, behavioral health, and social needs
  • Develop individualized care plans focused on social determinants of health including housing, transportation, food insecurity, and financial barriers
  • Coordinate behavioral health services including outpatient therapy, psychiatric care, crisis intervention, and substance use treatment
  • Collaborate with nurses, medical case managers, physicians, and interdisciplinary teams on integrated care plans
  • Connect members with community resources, government assistance programs, and social service organizations
  • Provide crisis intervention support and assist with safety planning during behavioral health emergencies
  • Support transitions of care from inpatient psychiatric facilities, acute care settings, and residential treatment programs
  • Advocate for member access to medically necessary behavioral health and social support services
  • Maintain accurate documentation of assessments, interventions, and care coordination activities within the care management platform
  • Monitor member progress and adjust care plans as needed to support positive health outcomes
  • Promote trauma-informed and member-centered care approaches throughout interactions
  • Participate in care management meetings and interdisciplinary case reviews

Qualifications:

  • Master of Social Work (MSW) degree from an accredited program required
  • Active Licensed Clinical Social Worker (LCSW) licensure required
  • Multi-state licensure preferred
  • Minimum of 3 years of post-licensure experience in case management, behavioral health, or managed care required
  • Experience within a health plan, managed care organization, or population health setting preferred
  • Strong knowledge of community resources, behavioral health systems, and social determinants of health
  • Experience with crisis intervention, trauma-informed care, and safety planning required
  • Strong communication, advocacy, and care coordination skills

Schedule:

  • Full-time position
  • Standard business hours
  • Standard full-time hours per pay period

Salary Range: The salary range for this position is approximately $70,000 – $95,000 annually ($33.65 – $45.67 per hour), based on experience and qualifications.
