Location: Baltimore, MD (On-site)
We are seeking a Social Determinants of Health (SDOH) Navigator to connect high-risk members with community resources and social services that support their overall health and well-being for a shared services center. This position is a full-time, direct hire opportunity based onsite in Baltimore, MD. The role screens members for social needs, connects them with programs for housing, food, transportation, utilities, and financial support, and follows up to make sure each connection is made.
This role is well aligned for a compassionate, resourceful professional who understands that health is shaped by much more than clinical care. The ideal candidate builds strong relationships with community organizations, government agencies, and social service providers, brings empathy and dependable follow-through, and works well as part of a cross-functional care team both in the office and out in the community.
Responsibilities:
- Conduct SDOH screenings for high-risk members using validated assessment tools
- Assess member needs across housing, food, transportation, utilities, social connection, and finances
- Connect members with community resources and benefit programs that match identified needs
- Assist members with Medicaid, SNAP, LIHEAP, and local benefit applications
- Coordinate with care managers and social workers on complex cases
- Build relationships with community organizations, food banks, housing providers, and transportation services
- Follow up with members to confirm resources were connected and needs were met
- Document SDOH screenings, referrals, and member interactions in the care management platform
- Track screening completion rates, referral outcomes, and resource utilization metrics
Qualifications:
- Associate degree in Human Services, Social Work, Community Health, or equivalent experience
- 1 or more years of experience in community health, social services, or patient navigation
- Knowledge of community resources, government benefit programs, and social service systems
- Familiarity with SDOH frameworks and validated health screening tools
- Ability to communicate with empathy and respect with members facing financial, health, and social challenges
- Proficiency with computer systems, data entry, and care management platforms
Schedule:
- Full-time position
- Onsite in Baltimore, MD, with regular engagement out in the community
- Monday through Friday, standard business hours (Eastern Time)
- 40 hours per week
Salary Range: The salary range for this position is approximately $42,000 – $60,000 annually ($20.19 – $28.85 per hour), based on experience and qualifications.
