Job Description

Location: Austin, TX (On-site)

We are seeking a Medical Social Worker, MSW to deliver psychosocial assessment, crisis support, and safe discharge planning for patients and families at an integrated delivery network. This position is a full-time, direct hire opportunity based onsite in Austin, TX. The role works as part of an interdisciplinary care team, providing direct support on the unit and at the bedside in a fast moving clinical setting.

This role is well aligned for a licensed social worker who is committed to whole person care and supporting patients through critical health events. The ideal candidate brings strong assessment and crisis intervention skills, collaborates easily with nurses, physicians, and case managers, and connects patients with the resources they need for a safe transition home or to the next level of care.

Responsibilities:

  • Conduct psychosocial assessments for newly admitted patients to identify social, emotional, financial, and safety needs
  • Develop and implement care plans that support treatment, recovery, and safe discharge
  • Coordinate post-acute placements and community services, including home health, skilled nursing, hospice, and outpatient behavioral health
  • Provide crisis intervention, supportive counseling, and emotional support to patients and families
  • Assess for domestic violence, substance use, mental health concerns, and abuse or neglect, with appropriate reporting
  • Collaborate with case managers, physicians, nurses, and discharge planners on complex discharge needs
  • Connect patients with financial assistance programs, insurance options, and community resources
  • Facilitate family meetings and advance care planning conversations as needed
  • Document social work assessments and interventions accurately in the electronic health record

Qualifications:

  • Master of Social Work (MSW) from a CSWE accredited program required
  • Active Texas LMSW or LCSW license required
  • 2 or more years of medical or healthcare social work experience preferred
  • Experience in acute care, case management, or behavioral health settings strongly preferred
  • Strong knowledge of community resources, post-acute care options, and social services systems
  • Excellent communication, crisis intervention, and interdisciplinary collaboration skills

Schedule:

  • Full-time position
  • Onsite in Austin, TX
  • Monday through Friday, standard business hours (Central Time)
  • Rotating on-call coverage and occasional weekend coverage based on patient census

Salary Range: The salary range for this position is approximately $62,000 – $76,000 annually ($29.81 – $36.54 per hour), based on experience and qualifications.
