We are hiring for a Home Health RN Case Manager with a Medicare-certified home health agency located in the US. This position is a full-time, direct hire opportunity.

Responsibilities:
- Conduct initial and recertification OASIS assessments to establish appropriate episodes and reimbursement levels.
- Develop and implement individualized plans of care aligned with physician orders and CMS requirements.
- Perform skilled wound care including debridement, NPWT, and complex dressing changes with documented measurements.
- Administer and monitor IV therapy, PICC lines, TPN, and injected medications with adverse reaction reporting.
- Manage medication reconciliation, identify polypharmacy risks, educate on adherence, coordinate adjustments with pharmacy and physicians.
- Coordinate interdisciplinary team services including PT, OT, SLP, social work, and home health aides.
- Communicate patient changes and hospitalization risks to physicians and specialists; document verbal orders timely.
- Monitor for clinical deterioration using risk tools, implement care transition protocols, coordinate urgent interventions.
- Complete clinical documentation including visit notes, OASIS, 485 plans, and discharge summaries per CMS standards.
Qualifications:
- Hold active, unrestricted RN license in Georgia; multi-state compact licensure preferred.
- Demonstrate 3+ years nursing and 1-2 years home health, case management, or post-acute care.
- Demonstrate OASIS competency or commit to completing training within 30 days; OASIS-E preferred.
- Perform skilled home health procedures including wound care, IV therapy, catheter management, and point-of-care testing.
- Understand CMS Conditions of Participation, Medicare coverage criteria, and home health documentation requirements.
- Demonstrate experience with home health EMR platforms; proficient with electronic documentation and clinical software systems.
Schedule:
- Full-time, Monday through Friday
- Field-based in the Atlanta, GA metro area
- Day shift with occasional on-call rotation
Salary Range: The salary range for this position is approximately $70,000 – $90,000 annually ($33.65 – $43.27 per hour), based on experience and qualifications.
