Job Description
Location: Atlanta, GA
We are seeking a Home Health RN Case Manager to manage a field-based caseload of patients receiving skilled nursing services through a Medicare-certified home health agency. This position is a full-time, direct hire opportunity based in the Atlanta, GA metro area.
This role is best aligned for an experienced RN who wants clinical autonomy and one-on-one patient relationships while practicing at the full scope of nursing. You will own your patients from admission through discharge: completing OASIS assessments, building the plan of care, delivering skilled interventions in the home, and coordinating the interdisciplinary team around each episode. The ideal candidate is self-directed, highly organized, confident with OASIS documentation, and driven by helping patients recover and stay safely at home.
Responsibilities:
- Conduct initial and recertification OASIS assessments to support appropriate episodes of care and reimbursement accuracy
- Develop and implement individualized care plans aligned with physician orders and CMS requirements
- Perform skilled wound care including complex dressing changes, NPWT support, and documented wound measurements
- Administer and monitor IV therapy, PICC lines, TPN, injections, and medication-related patient responses
- Complete medication reconciliation, identify adherence concerns, and coordinate updates with physicians and pharmacy teams
- Coordinate interdisciplinary services including PT, OT, SLP, social work, and home health aide support
- Communicate patient changes, hospitalization risks, and care needs to physicians and specialists
- Monitor for clinical deterioration and coordinate timely interventions or care transitions as needed
- Provide patient and caregiver education on disease management, medication use, and safe home care techniques
- Complete visit notes, OASIS documentation, 485 plans of care, and discharge summaries according to CMS standards
Qualifications:
- ADN required; BSN preferred
- Active, unrestricted RN license in Georgia required; multi-state compact licensure preferred
- Minimum of 3 years of nursing experience required
- Minimum of 1 to 2 years of home health, case management, or post-acute care experience preferred
- OASIS competency required, or ability to complete training within 30 days of hire; OASIS-E experience preferred
- Experience with skilled home health procedures including wound care, IV therapy, catheter management, and point-of-care testing
- Working knowledge of CMS Conditions of Participation, Medicare coverage criteria, and home health documentation requirements
- Experience with home health EMR platforms and electronic clinical documentation preferred
- Valid driver’s license, reliable transportation, and current auto insurance required for field-based visits
- Strong time management, independent judgment, and patient education skills
Schedule:
- Full-time position, Monday through Friday
- Field-based across the Atlanta, GA metro area
- Day shift with occasional on-call rotation
Salary Range:
The salary range for this position is approximately $82,000 – $98,000 annually ($39.42 – $47.12 per hour), based on experience and qualifications.
