Location: Phoenix, AZ (Hybrid)

We are seeking a Cardiac Rehabilitation Coordinator to guide patients through individualized recovery following cardiovascular events within an integrated delivery network. This position is a full-time, direct hire opportunity based in the Phoenix, AZ area on a hybrid schedule.

This role is best aligned for a clinician who works at the intersection of cardiovascular medicine and health coaching and wants real ownership of patient outcomes. The ideal candidate can build individualized exercise and education plans independently, monitor patients confidently during supervised sessions, and partner daily with cardiologists, advanced practice providers, dietitians, and behavioral health specialists.

Responsibilities:

  • Conduct comprehensive initial assessments for new cardiac rehabilitation patients using AACVPR and ACC/AHA guidelines
  • Design individualized exercise prescriptions incorporating aerobic, resistance, and flexibility components per physician orders
  • Supervise Phase II and Phase III exercise sessions, monitoring vital signs and telemetry to detect arrhythmias and clinical changes
  • Deliver structured education on nutrition, medication adherence, smoking cessation, stress reduction, and warning sign recognition
  • Facilitate behavioral counseling on lifestyle modification, return to work planning, and psychosocial adjustment
  • Collaborate with the cardiology team to align rehabilitation goals with the overall care plan and communicate clinical updates
  • Maintain accurate and timely documentation in the EHR, including session notes, progress summaries, and discharge planning
  • Track program outcomes including functional capacity improvements, risk factor modification, and readmission rates for reporting
  • Coordinate scheduling, referrals, insurance authorizations, and discharge planning for Phase II to Phase III transitions
  • Serve as a clinical resource to staff and patients on cardiac risk stratification and emergency response protocols

Qualifications:

  • Associate degree required; bachelor’s degree in nursing, exercise physiology, or a related clinical field preferred
  • Active licensure as an RN, RRT, Exercise Physiologist, or PT required
  • Current ACLS certification required and maintained throughout employment
  • AACVPR CCRP certification required within 18 months of hire
  • Minimum of 2 years of cardiac rehabilitation, step-down, ICU, or outpatient cardiovascular experience required
  • Phase II program coordination experience preferred
  • Experience documenting in a major EHR platform preferred; cardiac rehab outcomes registry tracking preferred
  • Proficiency with 12-lead ECG interpretation, telemetry monitoring, stress testing, and hemodynamic assessment
  • Strong patient education, motivational coaching, and interdisciplinary communication skills

Schedule:

  • Full-time position
  • Hybrid schedule with a combination of on-site days in the Phoenix, AZ area and remote days
  • Day shift, outpatient clinic hours, Monday through Friday

Salary Range: The salary range for this position is approximately $70,000 – $92,000 annually ($33.65 – $44.23 per hour), based on experience and qualifications.
