Location: Round Rock, TX
We are partnering with a respected healthcare organization in Round Rock, Texas, to hire a qualified and compassionate Care Coordinator. This full-time, on-site role plays a critical part in delivering high-quality, patient-centered care by facilitating communication, managing care plans, and ensuring continuity across healthcare services. Responsibilities:
- Coordinate care plans in partnership with patients, families, providers, and community resources
- Serve as a liaison between patients and care teams to ensure communication is clear, timely, and actionable
- Facilitate referrals, prior authorizations, follow-up appointments, and transitions of care across settings
- Assess patient needs and address barriers to care, including transportation, insurance, or social determinants of health
- Educate patients about diagnoses, medications, treatment plans, and next steps to support informed decision-making
- Document all care coordination activities accurately and promptly in the electronic health record (EHR)
- Participate in interdisciplinary team meetings and contribute to process improvement efforts
- Ensure compliance with healthcare regulations, payer requirements, and organizational protocols
Qualifications:
- Bachelor’s degree in Nursing, Social Work, Public Health, or Healthcare Administration preferred
- Minimum of 2 years of experience in care coordination, case management, or patient services
- Familiarity with Medicare, Medicaid, and commercial insurance programs
- Proficient in EHR platforms and Microsoft Office applications
- Strong communication, organization, and follow-through skills
- Ability to multitask and manage competing priorities in a fast-paced environment
- CCM (Certified Case Manager) or similar certification preferred, or willingness to obtain within 12 months
Schedule & Work Environment:
- Full-time, Monday through Friday
- Standard business hours: 8:00 AM – 5:00 PM
- On-site position based in Round Rock, TX
- Collaborative, community-focused facility that values teamwork, patient advocacy, and service excellence
- No weekend or on-call responsibilities
Salary Range: The salary range for this position is $42,000 – $50,000 per year ($20.00 – $24.00 per hour), depending on experience and qualifications.
