Location: Dallas, TX (Onsite)

We are seeking a Discharge Planning Coordinator, RN to manage safe, timely transitions out of a high-volume acute care hospital. This position is a full-time, direct hire opportunity based on-site in Dallas, TX.

This role is best aligned for an acute care RN who wants to work at the intersection of clinical judgment, payer navigation, and post-acute placement. You will assess patients within 24 hours of admission, build the discharge plan around what each patient actually needs at home, secure authorizations, and clear the barriers that hold up a safe discharge. The ideal candidate moves quickly in a high-census environment, knows post-acute coverage criteria cold, and can hold their own with physicians during daily rounds.

Responsibilities:

  • Initiate discharge assessments within 24 hours of admission, screening for complexity, social risk, functional limitations, and post-acute needs
  • Develop individualized discharge plans addressing clinical, psychosocial, functional, and logistical needs, updating them as the patient’s status changes
  • Coordinate placements to skilled nursing facilities, inpatient rehabilitation, home health, hospice, and assisted living
  • Obtain and manage prior authorizations for post-acute services with payers and utilization management vendors
  • Collaborate with physicians to align discharge timing with clinical milestones and obtain required certifications
  • Participate in daily rounds and case conferences, presenting discharge plans and coordinating across disciplines
  • Identify and resolve discharge barriers including transportation gaps, limited caregiver support, unsafe home environments, and coverage issues
  • Facilitate social work referrals for financial assistance, guardianship, and community benefit programs
  • Coordinate durable medical equipment, infusion services, wound supplies, and home oxygen setup prior to discharge
  • Document discharge planning activity, authorization status, and avoidable delay days in the electronic health record

Qualifications:

  • ADN required; BSN preferred
  • Active, unencumbered RN license in Texas required; multi-state compact licensure preferred
  • Minimum of 3 years of acute care nursing experience required
  • Minimum of 1 year of discharge planning, case management, or utilization review experience required
  • CCM or ACM certification preferred; non-certified candidates expected to complete certification within 18 months of hire
  • Working knowledge of Medicare, Medicaid, and commercial coverage criteria including SNF, home health, and IRF eligibility requirements
  • Experience with Epic, Cerner, or Meditech, along with payer portals and prior authorization platforms
  • Strong prioritization, interdisciplinary communication, and patient and family engagement skills

Schedule:

  • Full-time position, Monday through Friday, with rotating on-call coverage
  • On-site in a Dallas, TX hospital setting
  • Occasional weekend coverage based on patient volume

Salary Range:

The salary range for this position is approximately $82,000 – $98,000 annually ($39.42 – $47.12 per hour), based on experience and qualifications.
