Job Description
Location: Dallas, TX (Onsite)
We are hiring for a Discharge Planning Coordinator, RN with a high-volume acute care hospital located in Dallas, TX. This position is a full-time, direct hire opportunity.
Responsibilities:
- Initiate discharge assessments within 24 hours screening for complexity, social risk, functional limitations, and post-acute needs.
- Develop individualized discharge plans addressing clinical, psychosocial, functional, and logistical needs with real-time updates.
- Coordinate placements to skilled nursing, inpatient rehab, home health, hospice, and assisted living.
- Obtain and manage prior authorizations for post-acute services with payers and utilization management vendors.
- Collaborate with physicians to align discharge timing with clinical milestones and obtain certifications.
- Participate in daily rounds and case conferences presenting plans and coordinating across disciplines.
- Identify and address discharge barriers including transportation gaps, caregiver support, unsafe homes, and insurance gaps.
- Facilitate social work referrals for financial assistance, guardianship, domestic violence, substance abuse, and benefits.
- Coordinate durable medical equipment, infusion services, wound supplies, and oxygen setup before discharge.
Qualifications:
- Maintain active, unencumbered RN license in Texas; multi-state compact licensure preferred
- Hold BSN or ADN with discharge planning experience; commit to BSN completion
- Provide 3+ years acute care nursing; 1+ year discharge planning, case management, or utilization review
- Obtain CCM or ACM certification; non-certified candidates must complete exam within 18 months
- Demonstrate Medicare, Medicaid, and commercial insurance coverage criteria; SNF, home health, IRF eligibility
- Use EHR systems including Epic, Cerner, or Meditech; navigate payer portals and prior authorization platforms
Schedule:
- Full-time, Monday through Friday with rotating on-call coverage as needed
- On-site in Dallas hospital setting
- Occasional weekend coverage may be required based on patient volume
Salary Range: The salary range for this position is approximately $68,000 – $88,000 annually ($32.69 – $42.31 per hour), based on experience and qualifications.
