Location: Philadelphia, PA

We are hiring for a Health Equity Program Coordinator with a mission-driven community health system located in the US. This position is a full-time, direct hire opportunity.

This role is ideal for a public health professional who is energized by community-facing work and wants to see the direct impact of their efforts in the neighborhoods they serve. You will sit at the intersection of data, community partnership, and clinical systems, translating equity goals into coordinated, on-the-ground action. Whether you are managing grant deliverables, facilitating partnerships with FQHCs and community-based organizations, or leading staff training on culturally responsive care, you will play a visible and valued role in shaping how this health system shows up for its communities. 

Responsibilities:

  • Design and coordinate health equity programs addressing housing, food insecurity, transportation, and health literacy barriers.
  • Analyze patient data on demographics, health outcomes, and utilization to identify disparities and track equity progress.
  • Develop partnerships with community organizations, FQHCs, faith institutions, and social service agencies for referral networks.
  • Lead community health events and mobile screenings, managing logistics, volunteers, vendors, and post-event data collection.
  • Support grant applications and funder reports by synthesizing program data and writing narrative documentation.
  • Coordinate community health needs assessments including stakeholder engagement, surveys, data analysis, and findings presentation.
  • Develop and facilitate staff training on culturally competent care, implicit bias, trauma-informed practice, and language access.
  • Maintain the community resource directory ensuring current, accessible referral pathways aligned with patient population needs.
  • Integrate SDOH screening tools into care workflows with clinical teams to address identified social needs.

Qualifications:

  • Demonstrate 2-4 years of experience in health equity, community health, or health system program coordination.
  • Hold bachelor’s degree in public health, social work, or health administration; MPH or MSW preferred.
  • Demonstrate knowledge of SDOH frameworks, health disparities, community needs assessment, and equity-focused interventions.
  • Demonstrate ability to build trust with diverse, low-income populations across cultural and linguistic differences.
  • Master data management tools including Excel and REDCap; report and visualization skills preferred.
  • Know SDOH screening tools like PRAPARE and AHC HRSN; integrate social needs into EHR systems.

Schedule:

  • Full-time, Monday through Friday
  • On-site in Philadelphia, PA with community outreach activities
  • Standard business hours; occasional evenings for community events

Salary Range: The salary range for this position is approximately $52,000 – $68,000 annually ($25.00 – $32.69 per hour), based on experience and qualifications.
