Job Description

Location: Baltimore, MD (On-site)

We are seeking a Health Information Management (HIM) Specialist to oversee the full lifecycle of the medical record, ensuring documentation standards are met, records are coded accurately, and health information stays protected and accessible. This position is a full-time, direct hire opportunity based on-site in Baltimore, MD.

This role is best aligned for a detail-oriented HIM professional who knows the medical record end to end. You will manage record completeness and integrity in the EHR, process release of information requests under HIPAA, code outpatient encounters, resolve master patient index issues, and support the audits and accreditation reviews that depend on clean documentation. The ideal candidate works independently as an individual contributor and partners across departments to keep documentation standards consistent.

Responsibilities:

  • Manage patient health records in electronic health record systems to ensure completeness, accuracy, and timeliness
  • Process release of information requests in compliance with HIPAA regulations and organizational policies
  • Audit medical records for documentation deficiencies and coordinate physician completion activities
  • Assign ICD-10-CM, ICD-10-PCS, and CPT codes to outpatient encounters, clinic visits, and ancillary services
  • Monitor coding quality metrics and support coder education to address accuracy trends
  • Maintain master patient index integrity and resolve duplicate medical record number issues
  • Support medical record retrieval for audits, legal requests, quality reviews, and accreditation activities
  • Participate in EHR optimization projects related to documentation templates, forms, and record structure
  • Analyze data quality reports to identify and correct errors in patient demographic and clinical data
  • Maintain record retention and destruction schedules in accordance with state and federal requirements

Qualifications:

  • Associate degree in Health Information Management or a related field required; bachelor’s degree preferred
  • RHIT or RHIA credential required; CCS or CPC credential preferred
  • Minimum of 2 years of experience in health information management, medical records, or outpatient coding required
  • Proficiency with EHR systems and encoder software required; Epic or Cerner experience preferred
  • Strong working knowledge of HIPAA privacy and security regulations for protected health information
  • Experience with release of information processes and medical record auditing required
  • Strong attention to detail and cross-departmental communication skills

Schedule:

  • Full-time position
  • On-site in Baltimore, MD
  • Monday through Friday, standard business hours
  • Standard full-time schedule of 40 hours per week

Salary Range:

The salary range for this position is approximately $52,000 – $66,000 annually ($25.00 – $31.73 per hour), based on experience and qualifications.
