Location: Dallas, TX
We are seeking a Long-Term Care Pharmacist to support clinical pharmacy services across skilled nursing and long-term care facilities within the Dallas metro area. This role is responsible for medication regimen review, consultant pharmacy services, regulatory compliance support, and collaboration with interdisciplinary teams to improve medication safety and patient outcomes in geriatric populations.
This position is ideal for a pharmacist with long-term care or consultant pharmacy experience who enjoys working directly with care teams, supporting quality initiatives, and optimizing medication therapy for medically complex residents.
Responsibilities:
- Conduct monthly and quarterly medication regimen reviews for long-term care residents
- Identify medication-related concerns and provide written clinical recommendations to facility teams and prescribers
- Provide consultant pharmacist services in accordance with OBRA-87 and CMS long-term care requirements
- Collaborate with physicians, Directors of Nursing, and facility leadership on formulary management and medication policies
- Review medication administration records, physician orders, and pharmacy documentation for accuracy and compliance
- Participate in QAPI meetings and present medication utilization, adverse drug event, and quality trend data
- Monitor high-risk medications using Beers Criteria and geriatric pharmacotherapy best practices
- Evaluate medication profiles for new admissions, hospital discharges, and care transitions
- Support antibiotic stewardship, fall prevention, and deprescribing initiatives across assigned facilities
- Assist facilities with regulatory survey readiness and medication-related compliance initiatives
- Communicate clinical recommendations effectively with nursing staff, prescribers, and administrators
- Maintain compliance with organizational, state, and federal pharmacy regulations
Qualifications:
- Doctor of Pharmacy (PharmD) degree required
- Active Texas pharmacist license required
- Minimum of 3 years of long-term care, consultant pharmacy, or skilled nursing pharmacy experience preferred
- BCGP or CGP certification preferred or willingness to pursue certification strongly encouraged
- Knowledge of OBRA-87, CMS Conditions of Participation, and long-term care pharmacy regulations required
- Familiarity with Beers Criteria, deprescribing practices, and geriatric medication management principles
- Strong communication and relationship-building skills with interdisciplinary healthcare teams
- Ability to work independently in a field-based environment while managing multiple facility relationships
Schedule:
- Full-time, field-based position
- Monday through Friday day shift schedule
- On-call rotation participation required
Salary Range: The salary range for this position is approximately $110,000 – $135,000 annually ($52.88 – $64.90 per hour), based on experience and qualifications.
