Location: Remote (US-based)

We are seeking a Population Health Program Manager to design, implement, and evaluate programs that improve health outcomes for defined patient populations. This remote leadership role drives strategy across chronic disease management, preventive care, and health equity initiatives using data-driven approaches and cross-functional collaboration that directly impact quality scores and member outcomes.

This is a strategic role for someone who thinks in populations, not just patients. You will design programs that reach thousands of members, measure outcomes that move organizational performance metrics, and lead the cross-functional teams that make population health work in practice, not just in theory.

Responsibilities:

  • Design and manage population health programs targeting high-risk, high-cost patient segments including chronic disease, behavioral health, and maternal health populations
  • Analyze claims, clinical, and social determinants data to stratify populations and identify intervention opportunities
  • Develop program performance metrics, evaluation frameworks, and ROI models for executive reporting
  • Partner with care management, quality, and network teams to align programs with value-based contract requirements
  • Manage vendor relationships for care management platforms, patient engagement tools, and community health resources
  • Lead cross-functional workgroups to address health disparities and advance health equity goals
  • Monitor industry trends, CMS innovation models, and evidence-based program designs for continuous improvement
  • Present program outcomes and strategic recommendations to senior leadership and client stakeholders
  • Develop patient engagement strategies including digital outreach, telephonic coaching, and community partnership programs
  • Track HEDIS, CMS Star Ratings, and NCQA quality measures as they relate to population health program performance
  • Manage program budgets and ensure interventions are cost-effective relative to medical cost savings
  • Collaborate with data analytics teams on predictive modeling for readmission risk, ED utilization, and care gaps
  • Support Medicare Advantage and Medicaid managed care population health reporting requirements
  • Develop and maintain program documentation including policies, workflows, clinical protocols, and outcome reports

Qualifications:

  • Master’s degree in Public Health, Healthcare Administration, or related field preferred; bachelor’s with equivalent experience accepted
  • Minimum 5 years of experience in population health, care management, or health plan program management
  • Knowledge of value-based care models, HEDIS measures, CMS Star Ratings, and risk adjustment methodologies
  • Experience with population health analytics platforms and care management systems
  • Strong project management and stakeholder engagement skills
  • Data-driven mindset with ability to translate insights into program strategy
  • Experience managing programs across Medicare, Medicaid, and commercial populations
  • Excellent presentation skills for executive and client-facing communications

Schedule:

  • Full-time position
  • Standard business hours
  • Standard full-time hours per pay period

Salary Range: The salary range for this position is approximately $95,000 – $115,000 annually ($45.67 – $55.29 per hour), based on experience and qualifications. Relocation assistance may be available for qualified candidates.
