Location: Work From Home – Applicants must reside in West Virginia
We are seeking an experienced Patient Care Liaison to support a national patient access program focused on improving therapy coordination and healthcare communication. This work from home position is ideal for a professional who demonstrates empathy, attention to detail, and a strong commitment to accuracy and patient care. The Care Liaison plays a key role in ensuring patients receive timely support throughout the prescription referral and access process while maintaining the highest standards of confidentiality and HIPAA compliance.
Responsibilities:
- Serve as a primary point of contact for patient referrals, documentation, and provider communication.
- Review and process incoming referrals with precision and compliance to program standards.
- Perform accurate data entry and maintain up-to-date patient and prescriber information in program systems.
- Manage outbound communications via phone, email, and fax to ensure timely responses and resolution of inquiries.
- Demonstrate professionalism and empathy in all interactions while adhering to HIPAA and privacy regulations.
- Collaborate with pharmacy operations, case management, and other internal departments to ensure coordinated patient support.
- Identify and escalate issues that may affect patient access, program performance, or compliance standards.
- Participate in process improvements and assist with assigned projects as needed.
Qualifications:
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Minimum of 2 years of experience in healthcare, customer service, or patient support.
- Strong written and verbal communication skills.
- Proven ability to handle sensitive and confidential information in full compliance with HIPAA.
- Proficiency in Microsoft Teams, Outlook, Word, and Excel.
- Experience with pharmacy operations, HUB programs, or patient access services strongly preferred.
- Certified Pharmacy Technician (CPhT) credential is a plus.
Schedule & Work Environment:
- Full-time, work from home position.
- Standard operating hours between 8:00 AM and 8:00 PM EST, Monday through Friday.
- Reliable high-speed internet connection and a private, HIPAA-compliant home workspace required.
Salary Range: The salary range for this position is $39,500 – $56,000 a year ($19.00 – $27.00 per hour), based on experience.
Interview Process: Selected candidates will participate in a multi-step interview process, which will include a screening call with TalentLNX and one or more virtual interviews with Senior Leadership.
