Location: Louisville, KY (Hybrid)
We are seeking a Population Health Nurse Manager to lead care management and population health operations within a value-based care infrastructure anchored in an integrated delivery network. This position is a full-time, direct hire opportunity based in the Louisville, KY area on a hybrid schedule.
This role is best aligned for a seasoned RN who works at the intersection of clinical excellence, team development, and data-driven strategy. You will build and lead the care coordination team, design the protocols that govern chronic disease and behavioral health management, and own the metrics that drive value-based contract performance. The ideal candidate brings both bedside credibility and a system-level lens, and turns HEDIS gap data into clear, executable care coordination workflows.
Responsibilities:
- Recruit, onboard, and supervise the nursing and care coordination team
- Oversee risk stratification using predictive analytics and EMR tools to segment patients and assign interventions
- Design and implement evidence-based care management protocols for chronic disease and behavioral health populations
- Drive outreach programs targeting HEDIS care gaps, preventive screenings, wellness visits, and specialist referral closure
- Lead care transitions work with discharge planners and SNF liaisons to reduce readmissions and ED utilization
- Collaborate with physicians, behavioral health, pharmacy, and social services on multidisciplinary care plans
- Monitor and report performance metrics including HEDIS rates, care gap closure, readmissions, ED utilization, and patient activation
- Serve as the primary clinical escalation point for complex cases, guiding care coordinators and physicians
- Connect patients with community resources addressing social determinants of health
- Support value-based contract performance reviews with clinical and operational leadership
Qualifications:
- BSN required; MSN, MPH, or an equivalent graduate degree strongly preferred
- Active, unencumbered Kentucky RN license required; multi-state compact eligibility preferred
- Minimum of 3 years of population health or managed care experience required, including at least 1 year in a supervisory capacity
- CCM certification required within 12 months of hire; disease management or care transitions certification preferred
- Proficiency with Epic, care management platforms, and population health analytics tools required
- Working knowledge of ACO, PCMH, Medicare Advantage, and Medicaid managed care models with HEDIS expertise required
- Strong team development, clinical judgment, and cross-functional communication skills
Schedule:
- Full-time position
- Hybrid schedule with a combination of on-site days in the Louisville, KY area and remote days
- Monday through Friday, standard business hours
Salary Range:
The salary range for this position is approximately $95,000 – $118,000 annually ($45.67 – $56.73 per hour), based on experience and qualifications.
