Job Description

Location: Irving, TX

TalentLNX is partnering with a reputable healthcare organization in Irving, Texas, to hire a qualified and compassionate Care Coordinator. This is a full-time, onsite role responsible for supporting patient-centered care delivery by coordinating services, facilitating communication between stakeholders, and ensuring a smooth healthcare experience for each individual served.

Responsibilities:

  • Coordinate patient care plans in alignment with providers, patients, families, and community resources
  • Act as a key liaison between patients and healthcare professionals to ensure clear, consistent communication
  • Facilitate referrals, authorizations, follow-up appointments, and transitions of care between services or settings
  • Assess patient needs and assist with resolving barriers to care, including social determinants of health
  • Provide patient education regarding diagnoses, medications, treatment plans, and follow-up instructions
  • Maintain timely and accurate documentation in the electronic health record (EHR)
  • Participate in interdisciplinary team meetings to support patient outcomes and contribute to process improvements
  • Support compliance with internal protocols and external healthcare regulations

Qualifications:

  • Bachelor’s degree in Nursing, Social Work, Public Health, or Healthcare Administration preferred
  • Minimum of 2 years of experience in care coordination, case management, or patient services
  • Knowledge of insurance programs including Medicare, Medicaid, and commercial plans
  • Proficient in EHR systems and Microsoft Office Suite
  • Strong interpersonal and communication skills, both written and verbal
  • Ability to manage multiple priorities in a fast-paced environment
  • CCM (Certified Case Manager) or similar certification preferred, or willingness to obtain within one year

Schedule 

  • Full-time, onsite
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Standard business hours, no weekend or on-call responsibilities

Salary Range: Compensation for this role ranges from $50,000 to $60,000 per year (approximately $24.00 to $29.00 per hour), depending on experience and qualifications.

You may be asked to walk through scenario-based examples that reflect day-to-day responsibilities.

  • Final Interview – Onsite Meeting with Key Team Members
    An in-person meeting with 1–2 individuals from the care coordination or clinical team. Because where you work—and who you work with—matters just as much as the work itself, this final conversation helps ensure a strong mutual fit in terms of team dynamics, communication, and shared values.
