Job Description

Location: Raleigh, NC

We are looking for an experienced and knowledgeable Clinical Pharmacist to provide expert medication management and support optimal patient care. The Clinical Pharmacist will be responsible for reviewing, preparing, and dispensing medications while ensuring compliance with state and federal regulations. This role will also involve direct collaboration with healthcare providers and patients to optimize therapeutic outcomes, educate on proper medication use, and address clinical concerns.

Responsibilities:

  • Review and verify medication orders for accuracy, safety, and appropriateness based on patient-specific factors.
  • Prepare and dispense prescribed medications in accordance with established protocols and regulatory guidelines.
  • Provide medication therapy management, including monitoring for efficacy and adverse effects.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust treatment plans.
  • Educate patients on medication regimens, including proper use, potential side effects, and adherence strategies.
  • Maintain accurate records of medications, patient interactions, and clinical interventions.
  • Conduct medication reconciliation during patient transitions to ensure continuity of care.
  • Stay up-to-date with advancements in pharmacology, treatment guidelines, and regulatory changes.
  • Assist in developing and implementing pharmacy policies, procedures, and protocols to enhance operational efficiency and patient safety.
  • Mentor pharmacy students or residents as needed and provide guidance to pharmacy technicians.

Qualifications:

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited program.
  • Active North Carolina pharmacist license in good standing.
  • At least 2-3 years of clinical pharmacy experience in a hospital, outpatient, or similar setting.
  • Strong understanding of pharmacokinetics, pharmacodynamics, and therapeutic drug monitoring.
  • Excellent problem-solving, communication, and interpersonal skills.
  • Proficiency with electronic health records (EHR) and pharmacy software systems.
  • Board certification in a pharmacy specialty (e.g., BCPS, BCACP) is a plus.

Salary Range: The salary range for this position is $110,000 –  $130,000 a year based on experience.
