Location: Memphis, TN (Hybrid)
We are seeking a Patient Care Coordinator to keep members engaged between visits and on track with their care plans for a third-party administrator. This position is a full-time, direct hire opportunity based in the Memphis, TN area on a hybrid schedule. The role connects members with primary care, specialists, and community resources through outreach, scheduling, social needs screening, and coordination with the clinical care management team.
This role is well aligned for a healthcare professional who builds genuine connections with people and enjoys helping them stay on top of their health. The ideal candidate listens closely, follows through consistently, and manages a personal panel of members as an individual contributor, splitting time between the Memphis office and remote work.
Responsibilities:
- Manage a panel of attributed members in a value-based care arrangement
- Conduct outreach to members due for preventive care and quality measure visits
- Schedule appointments with primary care providers, specialists, and community partners
- Capture and update social determinants of health data in the EHR
- Document each member interaction in the EHR within the same business day
- Monitor open care gaps and prioritize outreach by clinical and quality impact
- Coordinate transportation, pharmacy, and DME support for members who need it
- Connect high-acuity members with the clinical care management team through warm handoffs
- Send appointment reminders and confirm member follow-through
Qualifications:
- High school diploma or equivalent required
- 2 or more years of experience in a healthcare setting, such as scheduling, front office, medical assisting, or care coordination
- Working experience with at least one major EHR, such as Epic, Athena, eClinicalWorks, or NextGen
- Familiarity with HEDIS gaps, preventive care schedules, or value-based quality measures preferred
- Bilingual in English and Spanish strongly preferred
- Strong phone presence and empathetic communication skills
- Strong organizational skills to manage a panel of 60 to 100 active members
Schedule:
- Full-time position
- Hybrid schedule based in the Memphis, TN area, with a combination of onsite and remote days
- Monday through Friday, standard business hours (Central Time)
- 40 hours per week
Salary Range: The salary range for this position is approximately $40,000 – $55,000 annually ($19.23 – $26.44 per hour), based on experience and qualifications.
