Location: Jacksonville, FL (Hybrid)
We are seeking an RN Case Manager, Telephonic to manage a panel of complex, high-risk members and coordinate their care for a managed care organization. This position is a full-time, direct hire opportunity based in the Jacksonville, FL area on a hybrid schedule. The role builds individualized care plans, coordinates with primary care providers and specialists, and supports members through scheduled telephonic touchpoints, with clinical documentation aligned to InterQual or MCG standards.
This role is well aligned for a bedside RN who is ready to step away from the floor while continuing to make a direct impact on patient care. The ideal candidate builds trusting relationships with members, brings strong clinical judgment to care planning, and manages a personal member panel as an individual contributor, splitting time between the Jacksonville office and remote work.
Responsibilities:
- Manage a panel of 60 to 90 complex and high-risk members telephonically
- Complete comprehensive health and psychosocial assessments at intake
- Develop individualized, member-centered care plans with measurable goals
- Conduct scheduled telephonic touchpoints aligned to acuity and program guidelines
- Apply InterQual or MCG criteria where utilization decisions intersect with case management
- Coordinate care across primary care providers, specialists, behavioral health, and community resources
- Connect members with support for transportation, housing, food, and caregiver needs
- Provide member education on chronic conditions, medications, and self-management
- Document assessments, care plans, and progress in the case management platform
Qualifications:
- Associate degree in nursing required; Bachelor of Science in Nursing preferred
- Active, unrestricted RN license in state of residence; compact license preferred
- Certified Case Manager (CCM) certification, or willingness to obtain within 24 months
- Minimum of 3 years of clinical nursing experience
- Case management, utilization management, or discharge planning experience preferred
- Working knowledge of Medicare and Medicaid benefits and regulations
- Comfort with motivational interviewing and patient-centered care planning
Schedule:
- Full-time position
- Hybrid schedule based in the Jacksonville, FL area, with a combination of onsite and remote days
- Monday through Friday, standard business hours (Eastern Time)
- 40 hours per week
Salary Range: The salary range for this position is approximately $78,000 – $108,000 annually ($37.50 – $51.92 per hour), based on experience and qualifications.
