Location: Boston, MA (Onsite)
We are hiring for a Pediatric Care Coordinator with a pediatric health system located in the US. This position is a full-time, direct hire opportunity.
Responsibilities:
- Serve as primary coordinator for complex pediatric patients with proactive family touchpoints throughout care continuum.
- Conduct comprehensive assessments across clinical, psychosocial, educational, developmental, and financial domains using validated screening tools.
- Develop and maintain individualized care plans with multidisciplinary team, ensuring documented, measurable, family-approved goals.
- Coordinate and track appointments, referrals, testing, imaging, and consultations across internal and external networks.
- Facilitate communication across families, providers, specialists, inpatient teams, schools, and community organizations as central bridge.
- Navigate insurance benefits, prior authorizations, coverage appeals, and financial assistance programs including Medicaid and nonprofits.
- Monitor care plan progress and conduct proactive outreach for appointments, screenings, authorizations, and medication refills.
- Support critical care transitions (NICU discharge, hospital-to-home, ED follow-up, pediatric-to-adult handoffs) with complete clinical context.
- Document care coordination activities, assessments, updates, and communications in Epic per organizational and regulatory standards.
Qualifications:
- Hold Bachelor’s in Nursing, Social Work, Health Sciences, Public Health, or clinical/human services; Master’s preferred.
- Demonstrate 2+ years pediatric care coordination, case management, patient navigation, or clinical care management experience.
- Maintain active RN, LPN, MSW, LSW, LCSW, or equivalent licensure in Massachusetts.
- Demonstrate pediatric condition knowledge: asthma, diabetes, epilepsy, oncology, cardiac disease, autism, cerebral palsy, genetic disorders.
- Demonstrate Epic or EHR proficiency; manage care plans, referrals; population health software experience valued.
- Demonstrate MassHealth, CHIP, commercial managed care, specialty program knowledge; navigate prior authorizations and coverage appeals.
Schedule:
- Full-time, Monday through Friday, standard clinic hours
- On-site in Boston with potential for some hybrid flexibility after onboarding
- Occasional travel to satellite clinic locations within the health system
Salary Range: The salary range for this position is approximately $50,000 – $68,000 annually ($24.04 – $32.69 per hour), based on experience and qualifications.
