Location: Nashville, TN (Onsite)

We are hiring for a Value-Based Care Program Manager with a leading healthcare organization located in the US. This position is a full-time, direct hire opportunity.

This is a strategic leadership role ideal for a healthcare professional who combines deep knowledge of value-based payment models with strong program management and stakeholder engagement skills.

Responsibilities:

  • Manage the full lifecycle of value-based care contracts including ACO, MSSP, PCMH, and bundled payment programs
  • Develop and execute population health strategies to improve quality metrics and reduce avoidable utilization
  • Monitor program performance against quality benchmarks, financial targets, and contract requirements
  • Serve as the primary liaison between the organization and payer partners on VBC program operations
  • Collaborate with clinical, analytics, and finance teams to identify care gaps, risk cohorts, and cost drivers
  • Lead physician engagement initiatives to drive adoption of evidence-based care protocols and documentation practices
  • Oversee HEDIS, Stars, and other quality measure performance tracking and improvement activities
  • Prepare and present program performance reports for executive and physician leadership
  • Support contract negotiations and renewals with actionable performance data and strategic recommendations
  • Manage project plans, timelines, and cross-functional workgroups for VBC initiative implementation
  • Stay current on CMS innovation models, state Medicaid VBC programs, and commercial payer trends

Qualifications:

  • Bachelor’s degree in Healthcare Administration, Public Health, Business, or related field required; Master’s degree preferred
  • 3 or more years of experience in value-based care, population health, or managed care program management
  • Strong knowledge of ACO, MSSP, PCMH, bundled payments, and quality measurement frameworks
  • Demonstrated experience managing payer and physician relationships in a value-based environment
  • Proficiency with healthcare data analytics, reporting tools, and EHR data interpretation
  • Excellent project management, communication, and executive presentation skills

Schedule:

  • Full-time, Monday through Friday, standard business hours
  • On-site in Nashville with potential hybrid flexibility for senior-level work
  • Travel to partner sites and payer meetings as needed

Salary Range: This position offers a competitive salary of $90,000 – $120,000 annually based on experience and qualifications.
