Location: Raleigh, NC (On-site)

We are seeking an Inpatient Clinical Pharmacist to deliver comprehensive inpatient clinical pharmacy services for a leading health system. This position is a full-time, direct hire opportunity based onsite in Raleigh, NC. The role participates in daily interdisciplinary rounds, provides pharmacokinetic and therapeutic drug monitoring, and drives antimicrobial stewardship and medication safety initiatives across inpatient units.

Responsibilities:

  • Review inpatient medication orders for therapeutic appropriateness, drug interactions, contraindications, and patient parameters
  • Participate in daily interdisciplinary rounds, providing pharmacotherapy recommendations and formulary guidance
  • Perform pharmacokinetic monitoring and dosing adjustments for high alert and narrow therapeutic index medications
  • Lead medication reconciliation at transitions of care to resolve discrepancies and support safe handoffs
  • Identify, assess, and report adverse drug reactions and medication safety events
  • Provide discharge medication counseling on indication, dosing, side effects, and adherence strategies
  • Lead antimicrobial stewardship initiatives, including audits, de-escalation, IV to oral conversions, and duration reviews
  • Provide therapeutic drug monitoring and clinical consultation for complex cases
  • Respond to medical emergencies with medication support, dosing guidance, and clinical decision making

Qualifications:

  • Doctor of Pharmacy (PharmD) from an ACPE accredited college or school of pharmacy required
  • Active, unrestricted North Carolina pharmacist license, or commitment to obtain prior to hire
  • Completion of an ASHP accredited PGY-1 pharmacy residency required; PGY-2 specialty residency strongly preferred
  • BPS certification (BCPS, BCCCP, BCOP, or BCIDP), or commitment to complete the exam within 12 months
  • Minimum of 2 years of inpatient clinical pharmacy experience in acute care or an academic medical center
  • Strong EHR proficiency, with Epic pharmacy experience and familiarity with clinical decision support and pharmacokinetic tools

Schedule:

  • Full-time position
  • Onsite in Raleigh, NC
  • Rotating shifts, including weekends and holidays as required
  • On-call responsibilities based on department needs

Salary Range: The salary range for this position is approximately $125,000 – $150,000 annually ($60.10 – $72.12 per hour), based on experience and qualifications.
