Job Description

Location: Columbus, OH (Onsite)

We are seeking a Long-Term Care Consultant Pharmacist to conduct medication regimen reviews across a panel of skilled nursing and assisted living facilities and support safe, effective, and compliant medication use for residents. This position is a full-time, direct hire opportunity based in Columbus, OH. The role partners with attending physicians and nursing leadership on deprescribing, psychotropic reduction, and survey readiness on a consistent, regional review schedule.

This role is well aligned for a pharmacist who enjoys clinical consulting and building lasting working relationships with prescribers and facility teams. The ideal candidate delivers clear, well supported recommendations, earns the trust of physicians and nurses, and helps each facility maintain strong medication practices and survey readiness.

Responsibilities:

  • Perform monthly medication regimen reviews for residents across an assigned facility panel
  • Identify and document drug therapy problems, including duplication, interactions, and inappropriate dosing
  • Recommend deprescribing and psychotropic gradual dose reductions consistent with CMS requirements
  • Review antibiotic use and support facility antimicrobial stewardship programs
  • Evaluate medication use against Beers Criteria and facility clinical policies
  • Advise nursing leadership on medication administration practices, storage, and documentation
  • Support survey readiness, including F-tag preparation and response to medication related findings
  • Participate in quality assurance and performance improvement (QAPI) committee meetings
  • Review medication error and adverse drug event reports and recommend process improvements
  • Provide in-service education to nursing staff on medication safety topics
  • Maintain consultation documentation to state and federal standards
  • Coordinate with the dispensing pharmacy on formulary, availability, and delivery

Qualifications:

  • Doctor of Pharmacy or Bachelor of Science in Pharmacy from an accredited program required
  • Active, unrestricted Ohio pharmacist license required
  • Board Certified Geriatric Pharmacist (BCGP) preferred
  • Minimum of 3 years of pharmacy practice experience, with long-term care consulting experience preferred
  • Working knowledge of CMS long-term care regulations, F-tags, and the survey process
  • Familiarity with Beers Criteria, psychotropic reduction requirements, and antimicrobial stewardship
  • Strong consultative communication skills with prescribers and nursing leadership
  • Valid driver’s license and willingness to travel regionally between facilities required

Schedule:

  • Full-time position
  • Onsite at assigned facilities in the Columbus, OH region, with predictable regional travel
  • Monday through Friday, standard business hours (Eastern Time)
  • 40 hours per week

Salary Range: The salary range for this position is approximately $115,000 – $135,000 annually ($55.29 – $64.90 per hour), based on experience and qualifications.
